The Polis application was developed as a tool for monitoring chronic patients, namely those with arterial hypertension.This application focuses on the needs of the chronically ill, stimulating their autonomy, their knowledge in health and their community involvement in activities that promote healthy lifestyles.
With the Polis application the user will be able to record their health problems, chronic medication as well as register specific parameters for monitoring their disease such as height, body weight, blood pressure and heart rate.
